Some certificates are now available through e-mail. If the certificate
you are requesting is available electronically, the e-mail column on this request form will be enabled
with a check box. You must check the box to receive the certificate electronically. The default method
of delivery for all eligible certificates will be email.
If you would like the certificate to be mailed via the US Postal Service or picked up at 148 West River Street, Providence, RI,
please complete the Special Filing Instructions on the Certificate Request Form to indicate your preferred method of delivery.
All emailed certificates can be printed in color and are affixed with a unique certificate number and website
address where the certificate may be verified for authenticity.
Payment: All major credit cards are accepted.
Processing Time: Two business days (plus mailing time).
